Job DescriptionTo provide holistic care and support to the physical and psychosocial well-being of clients in the Day Activity CentreTo plan and execute daily activities for clients based on AWWA DAC?s programme, according to Person Centred Active Support.To provide assistance in activities of daily living (e.g. feeding, toileting, transfers etc.) as required by clients.To effectively communicate with Centre Supervisor and team members of AWWA DAC.To work within a multi-disciplinary team and family/caregivers to assess, identify, implement and execute services and activities to meet the needs of the respective clients through an individualised care support plan(ICSP).To retrieve and maintain accurate programme records of all clients in AWWA DAC in a timely manner.To attend meetings as and when required by the programme.To be responsible for the upkeep and maintenance of all programmeresources.To assist in ad-hoc projects/committees as and when required by the Centre Supervisor.Job RequirementsAt least a Diploma/Degree in Therapy or Exercise or Sports ManagementMinimum 2 years of relevant experience in with a social service agency or equivalent sectorFamiliar working with Adults with DisabilitiesIndependent and Team PlayerGood communication skills
